When the admission office accepts the application, candidates for transfer are requested to deliver the following documents:
1. Personal questionnaire
2. Application form together with a justification approved by a dean or another person in charge of the education process management at the university they are leaving
3. Original high school diploma and transcript of grades (if it's not included on the Diploma) with a translation into Polish or English,
4. Original transcript of records from the previous university (in English)
5. Non-native speakers of English are requested to present certificates proving knowledge of the English language.
All high school documents issued outside Poland should be validated by the Apostille or legalized in the Polish embassy (only for countries that are not part of the Apostille Convention) in the county where the document has been issued.
