You must complete all credits from 11 semesters according to the study curriculum and submit to the Dean's office the following documents:
• confirmation of payment for the diploma - 100 PLN
• veterinary oath
• authorization (if you are unable to pick the diploma in person, you can authorize another person to act on your behalf)
• application form to be issued a certified copy of the diploma and diploma's supplement translated into English
All documents mentioned above must be submitted to the Dean's office between 15.01.2024 and 19.01.2024.

Before submitting the documents, go to USOSweb and check the correctness of your results from 1-10 semesters and your data (name, surname).
Within 30 days from the last exam's result entered on USOS, the University prepares and issues the diploma with its certified copies and the diploma's supplements to the graduate.
You will be informed by the Dean's office when the diploma is finished.
A certificate of completion
When the diploma is not yet prepared, you can receive a certificate of completion that is valid until the day of receiving the diploma.
